Depression is mostly treated with medicinal drugs, which work by targeting the mood or emotional symptoms of the patient. However, in the last few years, the medical experts have also started to consider the depression related pains while prescribing medications for depression. Several studies are going on to find out suitable pain management procedures for depressed individuals. Here, we have discussed about the medications that are currently available for managing depression related pain.

Duloxetine:
This medication is basically a special SNRI or serotonin norepinephrine reuptake inhibitor; it functions by increasing the levels of the two chemicals norepinephrine and serotonin in our brain. Norepinephrine and serotonin are responsible for managing our mood and memory, controlling our emotions and ensuring that we get proper sleep. All these functions often get altered in individuals suffering from depression. According to medical experts, intake of duloxetine also does not allow pain signals to reach the brain; as a result of this, the pain is not felt.

Nefazodone:
According to medical experts, the brain paths normally responsible for signaling pain resulting in creation of increased attention towards the feelings of pain might get altered in patients with depression. As the brain chemicals norepinephrine and serotonin regulate the process of reception of the pain signals, nefazodone, which is also an SNRI antidepressant can effectively reduce depression related pains.

Desvenlafaxine:
This is one of newest medications used for the treatment of depression. Like the other two medications mentioned above, this one is also categorized as serotonin norepinephrine reuptake inhibitor. As a result, desvenlafaxine can also reduce depression related pains effectively. Several studies have been conducted for finding out the effects of desvenlafaxine on patients with depression. Majority of them have revealed that desvenlafaxine can successfully reduce depression related pain and other signs of depression.

Major depressive disorder sufferers would typically be having an unhappy or down-in- the-dumps mood or losses of interests or pleasures in day-to-day goings-on persisting for a minimum of a fortnight. Such mood should correspond to a variation from the individual’s regular mood. Vital performance or operation (socially, occupationally, educationally or others) might even be affected unfavourably by the mood switch, for example, an individual missing his job or college as a result of depression or has completely discontinued class attendance or being present at habitual social appointments.

Substance-caused depressive mood (like the use of drug, alcohols, and medicines) isn’t deemed as major depressive disorder, neither is one that is arising from common medical conditions. Usually diagnosis of major depressive disorder symptoms is tricky in cases wherein individuals have a past of hypomanic, manic or mix incidents (for instance, a bipolar disorder) or in case schizo-affective disorder is accountable for the depressive mood & isn’t imposed on a hallucination, schizophrenia or psychotic disorders. Classically major depressive disorder isn’t diagnosed when the individual is mourning due to a major loss in his/her life.

Major Depressive Disorder Symptoms

Clinical depression or major depressive disorder symptoms are typified by the existence of most of the following:

Down or dejected mood felt on majority of the day, almost on a daily basis as signified by personal account (for instance, sense of sadness or void) or observations by those around (for example, appearing weepy). Among kids & those in their adolescence, it might be typified as a bad-tempered or touchy mood.

Markedly lowered interests or pleasures in each or nearly every activity on majority of the day, almost daily.

Considerable weight reduction while the person isn’t on a diet or weight increase (for example, an alteration crossing five percent of body-weight during a month’s time), or plummet/ rise in appetite almost daily.

Insomniac or hypersomniac behaviour daily.

Psychomotor hyperactivity or psychomotor impairment almost daily.

Sense of triviality or excess or inapt guiltiness almost daily.

Reduced capability of thinking or concentrating or indecision, almost daily.

Recurring thought process related to dying (not merely fearful of death), recurring suicidal tendency sans a particular agenda or suicidal endeavour or a defined agenda to commit suicide.

Moreover, for diagnosing major depression, the signs & symptoms mustn’t be accounted more by decease of a near-&-dear one, the symptoms persisting for more than duo months or typified by notable functional impediment, morose obsession with insignificance, suicidal envisaging, psychotic signs or psychomotor impairment.

Psychotic depression develops when acute depressive sickness has a co-existent type of insanity like the person hallucinating, becoming delusional or some sever with realism.

Psychotic depression afflicts around 1 from every 4 individuals hospitalized for depression.

Clinical Depression vs. Psychotic Depression

Psychotic depression entails all the clinical (major) depression symptoms like hopelessness, worthlessness, helplessness along with the attributes of psychosis. For example, an individual suffering from psychotic depression might hallucinate (see or hear things which are non-existent) or become delusional (illogical thought process & apprehensions).

As per information provided by the NIMH, a psychotic depression sufferer has lost touch with realism, hearing sounds or might be having weird & unreasonable thoughts. For instance, he/she might be thinking that those around are capable of hearing what’s going on in his/her mind or harbour intentions of harming him/her or the sufferer may feel he/she is the Prime Minister of some country or some celebrity.

A psychotic depression sufferer might get irate without any obvious ground or the person might be spending too much time staying secluded or lounging about or being wakeful during nights. Such a person might overlook the way he/she looks by not taking baths or even change clothes or that individual might be tricky to speak to, possibly the sufferer hardly ever speaks or is speaking senseless things.

Schizophrenics or other mental sickness sufferers even would be experiencing psychosis. However psychotic depression sufferers are generally conscious that what they are thinking is untrue. Such people feel humiliation or shame about what they are thinking & attempt at hiding them thus making diagnoses of such form of depression trickier.

However identification is crucial as experiencing psychotic depression event augments the prospects of developing bipolar depression with recurrent incidents of psychotic depression, obsession & also suicides.

Psychotic Depression Symptoms

A psychotic depression sufferer would feel:

Agitated.

Anxious.

Feel constipated.

Hypochondriac.

Insomniac.

Become intellectually impaired.

Become physically immobile.

Behave insanely.

Psychotic Depression Treatment

Generally, psychotic depression therapy is offered in hospital settings so that such patients could be closely followed up by mental health experts. Diverse medicines are deployed for stabilizing patient’s moods inclusive of combos of antidepressant & anti-psychotic medicines.

Neurotransmitter-affecting antipsychotic medications facilitate inter-nerve cell communicating. Several neuroleptics/ anti-psychotic drugs are currently being deployed for this condition like:

Olanzapine.

Ziprasidone.

Risperidone.

Quetiapine.

These medicines carry distinctive side-effects, generally, however patients have been found to better tolerate these as compared to previous antipsychotic medications.

Psychotic depression treatment has immense efficacy and recovery among individuals generally noted within a year’s time, though they have to be continually followed medically. In case the drugs fail in working to curb depression & insanity, at times ECT or electroconvulsive therapy is deployed. It is vital that patients should be working closely with their doctors for finding medication with maximum efficacy and minimal side-effects. Since psychotic depression is rather grave, the perils of suicide too are soaring.

Females are about two-folds more susceptible than men to go into depression. Such a disparity is observed to persist across races, ethnicities & monetary divides. This is partly because of hormone-related aspects, especially PMS (pre-menstrual syndrome), postpartum depression, PMDD, hyperthyroidism & perimenopause depression.

In regards to symptoms and signs of depression in women, they are more marked as compared to males and would be experiencing culpability, excessive sleepiness, over-eating (particularly binging on carbs) & weight increase. Females even have a greater likelihood of ailing from SAD or seasonal affective disorder (depression in the wintry months because of less amounts of sunlight).

As per information furnished by the National Mental Health Association, around twelve million females from the U.S. sink into clinical depression annually and around 1 from every 8 females could anticipate developing clinical depression during some phase of their life.

Symptoms & Signs of Depression in Women

Signs of depression in women bear resemblance to the ones in major depression. Prevalent grievances comprise of:

Downbeat or depressive mood.

A depressed woman would feel irritable, restless, disconcerted or on-edge. Her tolerance thresholds would be less and even the most insignificant of things seem to hassle her.

Lost interests or pleasures garnered from tasks one earlier enjoyed. No interests in earlier hobby, social activity, past-time or sexual contact. General loss in feeling joyful & content.

Feeling guilty, worthless & hopeless. An austere viewpoint about everything getting worse & nothing could be done for improving the scenario. Women under depression also tend to self-loathe and are harsh critics about themselves for observed flaws & blunders.

Thinking or pondering about committing suicide or recurring thought process involving mortality.

Sleep troubles (excessive sleepiness or deficit of sleep). A depressed woman would particularly awaken in the wee hours of dusk or become a hypersomniac (oversleep). Variations in appetites & weight fluctuations – considerable weight reduction or increase – an alteration of over five percent of body weight in a month’s time.

Problems try to concentrate, issues with focus, decision-making or recollecting things.

Lacking energy & weariness. Sense of fatigue, sluggishness & feeling physically sapped. All-body heaviness might be felt & just miniscule chores would feel arduous or taking protracted periods for completing.

An augment in physical whinges like headache, back ache, muscle pains & abdominal discomfort.

Depression is a mood disorder typified by differing extent of feeling sad, disappointed, lonely, hopeless, guilty & self doubt that could be rather severe & persisting for protracted spans of time. Everyday tasks might become harder in doing; however, the person may yet be capable of coping with them. During this stage, though, bleak feelings could turn rather acute.

Depression disorders have been categorized as bipolar & unipolar. Simply mania or depression coupled with mania is a manifestation of bipolar depression. Contrarily, depression is a manifestation of unipolar depression. Chief unipolar depression symptoms are:

Intense weariness.

Sleep disturbance, earlier wake-up times, problems falling asleep & sense of tiredness after a regular night’s snooze.

Lasting feeling of anxiousness.

Sense of sheer desolation, despondency or feeling useless about all aspects.

Irritable behaviour & feeling physically exhausted.

The disorder isn’t because of direct physiologic effects of abusing drugs or medicines or other substances or a broad medical problem.

Related traits:

Incapable of concentrating.

Lack of libido.

Illogical fears like fearing mortality, suicidal thought process or fears of executing suicide.

Unipolar Depression Causes

The reason possibly differs across individuals. At times a depressive event could unexpectedly surface when all is going alright. In other situations, depression could have a direct relation to a major episode in life like loss of a dear one, going through a traumatic episode or combating incessant sickness. Other reasons for unipolar depression might include:

PMS (premenstrual) perinatal hormonal variations.

Hormonal deficits.

Alcoholism.

Dependence on drugs.

Unipolar Depression Diagnosis

Several conditions have analogous symptoms hence differentiating against the below mentioned conditions that have to be excluded is necessary for ascertaining a definite identification.

Dementia.

Phobias.

Obsessive Behaviour.

Stress/anxiety condition.

Sexual disorders.

Evading work through pretence of being debilitated (malingering).

Unipolar Depression Treatment

Likely therapy choices which a physician might offer prescription comprise of:

Antidepressants.

ECT (Electroconvulsive therapy).

Psychotherapy.

CBT (Cognitive behavioural therapy).

Lifestyle alterations such as dietetic intake, exercising & smoke cessation.

Antidepressant medications definitely might reduce the mounting depression and stress in one’s lifestyle. But are you aware of the side effects they can cause? Anti- depressants can mess up with the weight and make one obese. It is a personal experience. One of my friends suffered from brain epilepsy conditions and was prescribed some medications which she continued for around a year. By the end of the phase, she had gained as much weight as she originally had, i.e. doubled her weight. She was determined enough to lose weight with exercise and diet, and all such other drives, in just the time of 6 – 8 months and now she looks as attractive, with the slimmed figure, as before. But every one is not fortunate and resolute enough to reverse such ill effects of anti- depressant medications. It is better to prevent it than to suffer twice as much the depression, later on.

Weight gained after trying to cure depression with anti- depressants may be experienced as an added amount of weight to one’s already heavy body. The onset of depression and anxiety may have never followed in a previous loss in the weight. This is definitely a negative sign or side effect of the consumption of anti- depressants. Reasons of weight gain after or during a depression treatment may be any of the following:

Depression treatments may include medicines of the kind which induce the cravings of the victim’s favorite foods and sweets to divert him from depressing thoughts and feelings and just keep him busy by cheering his taste buds. This leads to weight gain.

The anti- depressant drugs may act in the direction of increasing the appetite of the person by obstructing the histamine receptors. An excessive appetite leads to over consumption of all kinds of foods, resulting in weight gain.

Anti- depressants are usually the drugs that cause some mischief with the brain cells so that they get some rest off the depressing thoughts. These kind of drugs alter the way in which the restoration of body fats are stored and used up, which can be one cause of gaining of weight.

During treatments of depression, both the victim and the doctors focus on the mental health rather than the physical health, of the person. Thus, activities like exercise and physical fitness measures are switched to negligence. There is a low calorie expenditure of the person’s body and the result is seen as an increased amount of weight.

In rare cases, depression can cause loss in weight of the depressed. But, gradually, healing the condition can lead to gain of the previously lost kilos.

Since the medications are treating the deadly conditions of depression, most victims are not able to notice the side effects of the treatments and they take over quickly and easily. But, not always is weight gain due to anti- depressants a bad sign. Sometimes, due to depression and the ill effects, the victim’s weight may be reduced due to symptoms like loss of appetite, sleep and rising stress. Increase in the weight of the victim after or during treatments may depict the restoration of the lost weight, which is a positive sign towards good health. But these cases are rare.

This weight gain side effect of anti- depressant medication is, fortunately, a reversible damage and strivings and changes can be done to what these drugs have made out of you. How, let us see that in the subsequent posts.

Answer: On the contrary, a favourable curative relationship is vital to CBT. A therapist would do several things for building up a deep association like working in collaboration with the client (deciding jointly on matters like the manner of spending the treatment session), asking feedbacks or asking the patient how things could be done in a different manner during the next session. The therapist would be conducting himself/herself in an authentic, warm, compassionate, keen and considerate manner. A therapist would endeavour at reducing the patient’s agony as swiftly as doable. Surprisingly, studies have shown that the association between the therapist and patient would appreciably strengthen when the client perceives that his/her therapist is of assistance, which means that the client sees himself/herself solving his/her issues and feel better.

Query: Is client motivation necessary for CBT?

Answer: Several clients are apathetic when they initially seek treatment. An indispensable part of CBT is assisting the client in setting goals he/she wants to achieve that mostly bolsters motivational levels. A therapist would adopt a gentle querying approach during client sessions for discovering if the issue is because of a practical worry (for instance, the treatment is quite costly), or the client’s thought process that other people are blameworthy and he/she does not need therapy or the therapist making a mistake (for instance, has excessively interrupted the client). Several therapists also find it beneficial to have the client jot down what he/she sees as benefits and downsides of seeking the therapy and the benefits-drawbacks of not seeking therapy.

Query: How does one discern whether the therapist has in fact practiced CBT?

Answer: One could gather additional information from the site www.beckinstitue.org to gain a deeper understanding about CBT or check out the site of the Academy of Cognitive Therapy for locating authorized therapists. Regrettably, several individuals are self-proclaimed CBT therapists when actually they have failed to employ even the basic principles of this form of therapy. Several of these therapists fail in setting and discussing agendas, sending a client home along with (to-do or to-remember things for the subsequent weeks) notes and asking for feedbacks during each session.

Query: Is it factual that CBT is basically a superficial treatment and does not assist a client in gaining insight?

Answer: A client needs to gain a proper understanding of himself/herself to be able to have any form of improvement from CBT. A client needs to identify the reason for him/her feeling distressed or acting in an unconstructive manner (that is converse to their objectives). The resolutions are associated to what they are pondering at that instant. But being able to gain that insight is necessary but not sufficient. The client needs to discern what is to be done with that insight at that time for improving their quality of existence.

The subsequent approach is assisting the client in evaluating their thought process that would allow him/her in appraising scenarios and issues in a more realistic manner. When a client is able to do that, he/she would then be able to act in a functional manner and moving in the direction of their objectives in life.

A client would also need to assess and alter his/her unconstructive and non-realistic convictions. A therapist would frequently be working at deep levels, for e.g., a client’s objectives are linked to locating greater meaning from life, existing in accordance to his/her adhered value-sets or to come to terms or accept unchangeable issues.

A client suffering from depression following a successful treatment with CBT experiences half the recurrence rate as compared to individuals undergoing treatment with just drugs.
